Medical Exam Forms - All Scouts going to the Summer Camp will need to fill out a Medical Form Class 1. Medical Form Class 2 is required every 36 months. All Adults over the age of 40 needs a Medical Form Class 3 completed every year. Download the forms here and view them with Acrobat Reader.
